TI-RADS score

The TI-RADS evaluation system is one designed for a unitary quantification of nodular formations discovered ultrasound in the thyroid gland.

Depending on the composition, echogenicity, shape, edge, hardness and components inside the thyroid nodule, the TI-RADS system guides the physician regarding the risk of malignancy of that formation.

The risk levels for malignancy are:

  • TIRADS 1: RISK 0%
  • TIRADS 2: 0% RISK
  • TIRADS 3: RISK <5%
  • TIRADS 4: RISK 20-75%
  • TIRADS- 4A – 20%
  • TIRADS- 4B – 50%
  • TIRADS- 4C – 50-75%
  • TIRADS 5: RISK> 75%

 

Node tracking according to TI-RADS score

  • TIRADS 1 follow-up at 1 year (if it is associated with autoimmune thyroid disease) or at 5 years
  • TIRADS 2 follow-up at 2 years, treatment with IOD supplements
  • TIRADS 3 follow-up at 1 year
  • TIRADS 4 follow-up at 6 months
  • TIRADS 5 followed at 3 months

 

Depending on the TI-RADS score, the fine AC bridge may or may not be required:

  • TIRADS 1 is not the case
  • TIRADS 2 is not required
  • TIRADS 3 confirmation of benignity, at nodules larger than 2-2.5 cm
  • TIRADS 4 nodule clarification, at nodules larger than 1.5 cm
  • TIRADS 5 confirmation of suspected malignancy in nodules larger than 1 cm
